Birth/Death Registrations

Parents may now complete the Statement of Live Birth on-line and electronically submit it directly to ServiceOntario's Office of the Registrar General. Births occurring in the City of Cambridge will no longer be registered with the municipality. Instead, parents are encouraged to use the Newborn Registration Service at www.serviceontario.ca/newborn/. This new service allows parents to electronically submit the Statement of Live Birth and submit applications for a birth certificate and social insurance number at the same time.
Travel Document
TRAVEL DOCUMENTS ARE NO LONGER AVAILABLE. IF PARENTS ARE REQUIRED TO TRAVEL WITH THEIR NEWBORN AND HAVE NOT APPLIED FOR A BIRTH CERTIFICATE AT THE TIME OF REGISTRATION, PLEASE CONTACT THE OFFICE OF THE REGISTRAR GENERAL AT 1-800-461-2156 OR VISIT www.serviceontario.ca
Death Registrations
For deaths that occur in the City of Cambridge, the funeral director or person responsible for completing the death information brings the documents to the Clerk's Office. The Division Registrar records the information and forwards the documents to the Registrar General at the Province of Ontario.
When this has been completed, you may apply for a Death Certificate. Application forms are available Click Here or at the City Clerk's Office and at other locations across the Province.
